There are 2 processes by which a school can submit this form. Master Eligibility Form (Adobe Acrobat Reader Format) 1. Download the form (Master_Eligibility.pdf) 2. Print the form 3. Fill in the form 4. Fax the form to the ASAA office (Fax: 907-561-0720 or 907-563-3739) Master Eligibility Form (Microsoft EXCEL format) 1. Download the form (Master_Eligibility.zip) Click to download in zip file format. Note: This file is in an archived format. Windows users can use "WinZip" to open the archived file.
